Teach the adulting skill every teen needs: filling out forms correctly. Students learn form vocabulary (DOB, MI, N/A, signature vs. print), then complete 9 realistic forms drawn just like the real thing: a personal information card, library card application, school emergency contact form, 2-page new patient registration, gym membership agreement, change of address request, envelopes, checks and an online account sign-up. Every form uses a fictional practice persona, so students never write their own personal information on classroom copies.
