Help students learn, say and write their own personal information, and keep it safe. This personal information life skills pack for middle and high school special education and transition classes takes students from tracing and copying their name, address, phone number and date of birth, to reading ID cards, filling out 5 realistic forms, sorting what is private and what is OK to share, and practicing what to do on a scam call. Every practice page uses fictional people, and blank spaces let each student personalize the skills safely.

How to use it

Teach it in 8 sessions of 30-45 minutes, or pull single pages for centers, small groups and IEP goal practice. Students practice with the fictional people first, then fill in the spaces marked with a lock with their own information and an adult's help (families choose what to include). Low prep: print, then cut apart the sort cards, task cards and wallet card. Safety advice follows guidance from the FTC and the Social Security Administration.
